Free Medical Emergency Data Card

Be Prepared for Emergencies

In an emergency, getting vital health information to medical personnel quickly could be critical. Your Medical Emergency Data Card keeps a comprehensive personal medical profile handy at all times. Carry it with you at work, on vacation, or just walking in your neighborhood. You'll feel more secure knowing emergency medical personnel will have access to your data they need to administer appropriate care when there's no time for delay.

  1. It's So Easy! Complete the attached Medical Data Form. You may want to ask your physician for assistance. Be sure to PRINT CLEARLY using BLACK INK only. Put all requested information on the form; separate papers cannot be accepted.
  2. Indicate the type of card(s) you prefer. Cards come in two convenient styles. The wallet-size card is the size of a credit card. The wear-about style is a half-size card that can be tied to shoelaces, pinned to clothing or swimsuits, or worn on a necklace.
  3. Mail in your completed Medical Data Form. Use the enclosed return envelope or mail to the address on the order form.
  4. You'll receive your Medical Emergency Data Card(s) in four to six weeks.
  5. Keep your card with you at all times. Carry your card with your driver's license where it will be more likely be found.

About the Medical Emergency Data Card

When you send in your Medical Data Form, it is photographed on microfilm and laminated in a durable plastic card. Your medical profile is then readily available for use by emergency medical professionals. The data is easily read by an ordinary 8-power magnifying glass or by standard hospital emergency room magnification equipment. All cards are tested for readability before being mailed.

Only the Medical Data Form will be copied onto microfilm. Separate papers or other forms cannot be accepted; be sure all information appears on the Medical Data Form.

As a member, you may order a free medical card each year. If you have a joint account, you also may order a free card for one of the additional account holders. Simply have him/her complete a separate Medical Data Form and return it with your order. Additional forms are available at any of our offices.

It's important that you update your card annually to ensure your data is current. You will receive a reminder and renewal form every 12 months. If you need to update your card more often, you may do so for only $5 per card.

Cards Are Available for Family Members, Too

You may want to protect your spouse, children and other family members by ordering Medical Emergency Data cards for them also. Cards are especially important for children because they rarely carry other forms of identification. In case of an accident, the Medical Emergency Data Card helps ensure that the family is quickly notified and that the child receives prompt and appropriate medical treatment.

Additional cards for you or your family members are only $5 each. Similar cards cost $8 to $20 from other sources. To order extra cards, simply complete an additional Medical Data Form for each individual. Extra forms are available from your Customer Service Representative at any of our offices.
